Symptom checker follow up survey

Symptom checker follow up survey

To help us understand the impact of our axial SpA symptom checker we are seeking views from those who completed the symptom checker to see if it is leading to a referral and diagnosis. It is a current gap in our knowledge.

To aid this we have set up a follow on survey for those who have previously completed the symptom checker.

We firstly want to thank those who have completed our online 8-point symptom checker over the past 12 months. We hope that the symptom checker helped you in your health journey.

As a reminder you can see the symptom checker here.

We are trying to build an understanding of the difference that the symptoms checker is having in our ambition to drive down diagnostic delay in axial SpA. We want your views, your experience and insight to help us in our understanding.

It would be great if you could provide your answers to the online survey below.

We would be very grateful if you were able to provide us with answers to the questions included within this questionnaire, to help us progress with our work to remove barriers to early diagnosis and drive improvements in axial SpA care services.
